A Senior Connection LLC assists in choosing the facility of your choice
The types of living situations may include, Independent Living\g Communities, Assisted Living, both larger and Board and Care Communities , Memory Care (Alzheimer's/Dementia) and Nursing Care Communities.
We conduct a non-medical assessment to determine the needs and desires of the senior, that includes the care level needed, the "ideal" location, and the financial situation. We also assist in obtaining the medical information needed when the right community has been found.
We get a medical release signed by either the senior or the Power of Attorney.
We help explore all financial assistance that may be available, such as LongTerm Care Policies, VA aid and attendance benefits, and savings.
We also help determine if opting for Medicaid should be considered.
We set tours with the family and communities. We will meet the family at the communities for tours. After choosing the best fit, an assessment is completed by the community.
We obtain the physician's orders, and usually follow the senior into the new community with a script from their Physician for occupational therapy and physical therapy. This is generally covered by Medicare, or an insurance company, at no cost to the senior. It is set through an "in-home" health company.
We have an extensive network of professionals assisting with elder care legal advice, financial planning, downsizing, estate sales, moving, and space planning for the new living area.
